Supported Community Living
Who is served:
Older and younger adults with a variety of disabilities who want to live independently in the community. Also, people with disabilities who could benefit from training & support in areas such as self-care, safety, budgeting, home maintenance, community integration, and social skills.
What is offered:
Training in independent living skills: Self-advocacy, self-care, social skills, home maintenance & housekeeping, and community. Also, helping individuals establish, find, and use a natural support network in the community.
Day or evening, weekdays or weekends.
In the person's home and in the community: anywhere in Black Hawk, Benton, Bremer, Buchanan, Butler, Cerro Gordo, Chickasaw, Fayette, Floyd, Grundy, Hardin, Marshall, Tama, and other counties (call to inquire if services are provided in your area).
Funded through: The Medicaid Home and Community Based Waivers (HCBS), Habilitation Services Waiver, County or Region Funding, or private pay.
